Church Road is a bespoke service for adults with acquired brain injuries, supporting up to 8 individuals. Our purpose is to aid people in their recovery through rehabilitation approaches
prioritising personal goals and aspirations regaining independence.

Our Support
The people we will support at Church Road will have access to a wide range of dedicated support encompassing goal led rehabilitation, recreational activities, educational, personal,
emotional and social skills - which are appropriate to their individual needs, and whose quality and effectiveness are monitored and assessed at frequent intervals to ensure goals
and objectives are met and changing support needs and preferences are quickly identified and addressed.

Every individual that we support will have an allocated key worker who will provide support within all aspects of their person-centred plan.

We are very experienced and skilled in supporting rehabilitation focused placements. We have a therapy room on site and will be working closely with neuro-therapies to include SALT, OT,
Physio and neuro-psychology.

Accommodation (rooms, ensuites, other facilities)
The home offers comfortable and spacious accommodation over two floors and has a lift providing access for wheelchair users and those with mobility needs. The home is newly
refurbished with 4 self-contained flats and 4 comfortable bedrooms with en-suite. One of the rooms is equipped with a ceiling hoist and two of the studios have adjustable worktops
installed in the kitchen.

The four flats will aid those that are working towards goals as a transitional step, working towards regaining independence whilst remaining in an environment where support is at hand
when needed, whilst preparing for a possible move back into a more independent setting in the community. Every aspect of the home is designed to promote independence as far as is
practical or desired.The comfortable communal areas which can be accessed by all individuals who we support includes a newly fitted large kitchen, a comfortable lounge, a large and bright dining room and conservatory.

The home has a spacious garden and a greenhouse for use by all.

Location
The convenient and central location of Church Road allows easy access to a bustling high street, which is a short walkable distance away. The high street has an array of amenities and
community services. These include: a local Leisure Centre, Medical Centre, Theatre, Community Hall, Fishing Lakes and Golf Course..

Transport Links:
Poole and Bournemouth town centres can be easily assessed by public transport. There are
bus stops providing access to an array of local areas within a short walking distance from
Church Road.
Our nearest main rail and coach stations are located in Bournemouth, 7 miles away.
King George’s recreation ground is located a couple of minutes away from the home providing a beautiful grassland which hosts Football, Tennis, Bowls and Cricket pitches as
well as recreational facilities with public conveniences and refreshments available within the Pavillion.
