About Us
Choices Home Care provides a wide range of person-centred care and support services to help individuals remain safe, comfortable and as independent as possible within their own homes. We support people with a variety of needs and tailor each package of care around the individual’s assessed needs, preferences, routines and desired outcomes.
Our services include personal care, assistance with washing, dressing, grooming and continence needs, together with support for individuals who require care to be provided in bed. We provide both single and double-handed care calls depending on the level of support required.
We also provide medication support, including medication administration, monitoring, prompting and assistance with medication ordering, in accordance with the individual’s care plan and agreed level of support.
Our care team can provide support with more complex day-to-day care needs, including catheter care, stoma care and monitoring of relevant equipment. Staff follow agreed care plans and procedures and report any concerns or changes in an individual’s condition appropriately.
In addition to personal and clinical support, Choices Home Care provides assistance with everyday activities that help people maintain their independence. This may include:
Shopping calls and assistance with essential shopping.
Light domestic duties to help maintain a safe and comfortable home environment.
Laundry support.
Sitting services and companionship for individuals who require supervision or support at home.
Social calls and social inclusion to help reduce isolation and promote emotional wellbeing.
Support to attend medical, health and other important appointments.
Assistance to access the local community and participate in activities that are important to the individual.
Monitoring of equipment and reporting any concerns or faults where required.
Provision and appropriate use of Personal Protective Equipment (PPE).
Choices Home Care also supports individuals who require end-of-life care. We aim to provide compassionate, dignified and respectful care, working alongside the individual, their family, healthcare professionals and other services involved in their care.
Our approach is centred around dignity, choice, respect, independence and safety. We recognise that every person is different, and our aim is to provide consistent, responsive and person-centred care that reflects what matters most to each individual.
Through regular monitoring and review of care needs, we aim to ensure that support remains appropriate and that any changes in an individual’s health, wellbeing or circumstances are identified and acted upon promptly.
