Divine Enterprise (UK) Ltd trading as Bluebird Care Hammersmith & Fulham delivers responsive, strengths-based and outcome-focused Community Services that support residents to regain confidence, rebuild daily living skills and maximise independence within their own homes and communities. Established in 2012 and CQC-registered since 2013, we have extensive experience delivering domiciliary, rehabilitation and complex community care, including hospital discharge support, dementia care, Tracheostomy and ventilation, PEG, Epilepsy, learning disability and autism support, physical disability support and Continuing Healthcare packages.
Our service model aligns with the Care Act 2014, Mental Capacity Act 2005, NICE reablement guidance, NHS “Making Every Contact Count” principles and Hammersmith & Fulham’s strengths-based approach to adult social care. Through our established local infrastructure, experienced leadership team, skilled workforce and robust governance arrangements, we successfully support residents with complex physical, cognitive and rehabilitation needs.
