About Us

Nightingale Supported Communities is a domiciliary care provider delivering high-quality, person-centred care to adults in their own homes. We support individuals to live safely, comfortably, and independently, while maintaining dignity, choice, and control over their day-to-day lives.

Our services are tailored to each person’s assessed needs and may include personal care, medication support, companionship, domestic assistance, and support with daily routines. We work with adults of all ages, including older people and individuals living with physical disabilities, learning disabilities, mental health needs, dementia, and long-term health conditions.

Care is delivered by trained, compassionate staff who are supported through ongoing supervision, competency checks, and regular training. All care is planned and reviewed in partnership with the individual, their family (where appropriate), and other professionals involved in their care.

We work collaboratively with families, local authorities, NHS partners, and hospital discharge teams to support safe transitions home and continuity of care. Our approach is flexible and responsive, ensuring support adapts as needs change.
