About Us

The Active Care in the Home division employs 1250+ colleagues. Our packages of care are tailored to each person’s care needs, preferences, and lifestyle. Through our care in the home services, we support children, young people and adults with a wide range of conditions, including: brain injury, spinal cord injury, neurological conditions and neurodegenerative diseases, respiratory conditions and gastrointestinal conditions.

We don’t just look after the clinical needs of the people we support. We look after the whole person, and that includes their personal, social and emotional needs.

Through a holistic and innovative approach to care in all our services, we place the individual at the heart of everything we do. We work really hard to understand everyone from the start, so that we can design a personalised care plan that’s aligned with all their needs and preferences, and unlocks their potential.

We offer a flexible delivery model providing complex support services for Adults and Children nationally. This includes Live-In care, 24-hour support, wake, and sleep night services. This means that we are able to:

• Support the transition of Children and Younger People to an Adult service without a break in their care package.
• Relocate with the client and provide continuity of care without the need for a new provider.
• Offer both Live-In and Live-Out support packages and adjust the package to best meet client needs and outcomes.
• Provide experts by experience in traumatic injury; supporting the physical and psychological needs of the client and family

We provide progressive, personalised pathways of care that combine clinical expertise, personal support, and therapeutic and rehabilitation services.

We enable our clients to live the most rewarding lives possible in their own home.

All services are Registered to support TDDI (Treatment of disease, disorder or injury), our packages are overseen by a qualified clinician, care manager and dedicated care co-ordinator with systems allowing daily transparency of care records, incidents and supporting documentation.

About The Team
