About Us

Excel24/7 Healthcare Ltd provides nurse-led, trauma-informed home care for adults (18+) with complex, long-term, or high-risk health needs. We deliver the regulated activity Treatment of Disease, Disorder or Injury (TDDI) — providing clinical care at home, overseen by qualified nurses.

Our mission: Keep people safe, stable, and independent at home, while supporting smoother hospital discharge, step-down care, and hospital avoidance.

How We Work
Nurse-Led Oversight – Every package is clinically supervised. Medication safety, risk management, and professional governance are core. Staff receive continuous competency checks.

Trauma-Informed Practice – Care plans co-produced with individuals and families to promote dignity, reduce distress, and improve outcomes.

Digitally Enabled Care – Real-time monitoring of training, compliance, and outcomes ensures transparency and accountability.

Outcome-Focused Support – Clear goals for stability, independence, and safer care at home. Internal measures show 90%+ of clients remain safely at home without readmission.

Strong MDT Integration – We work closely with GPs, district nurses, therapists, pharmacists, and community teams to ensure joined-up care.

We are actively building relationships with local hospitals, ICBs, and discharge teams to:
- Reduce avoidable admissions and readmissions
- Support step-down and post-discharge care
- Provide rapid mobilisation for urgent or complex cases
