Oak Support Limited is a CQC-registered and CQC-rated Good domiciliary care provider delivering high-quality personal care and support to adults aged 18 and over in the comfort and familiarity of their own homes.

We have a team of caring, skilled, knowledgeable, and experienced care professionals who deliver compassionate and person-centred support tailored to individual needs, preferences, and aspirations. Our approach places each individual at the centre of their care, ensuring dignity, independence, choice, and wellbeing remain our priority at all times.

We support a wide range of individuals including older people, people living with dementia, mental health conditions, learning disabilities, autism, physical disabilities, and those requiring post-hospital discharge support, reablement, palliative care, or end-of-life care. Support can be provided on an ad hoc basis, through regular visits, overnight care, or as part of a comprehensive 24-hour package.

Oak Support Limited welcomes referrals from local authorities, NHS teams, hospitals, discharge coordinators, social workers, case managers, and privately funded service users. Our rates are competitive, transparent, and tailored to individual care requirements following an initial assessment.

Leadership and Professional Expertise

Our Registered Manager, Ms Chineme Duroha, is a Nursing and Midwifery Council registered nurse with more than 20 years of experience across clinical cancer care, community nursing, residential services, reablement, assessment, training, and health and social care management. Her extensive clinical expertise, leadership capability, and regulatory knowledge underpin the high standards delivered by Oak Support Limited.

Our Director, Ms Onyekachi Duroha, brings over 25 years of senior business experience across corporate strategy, project management, human resources, compliance, training, and business development. She is a Chartered Member of the Chartered Institute of Personnel and Development, a PRINCE2 Project Management Practitioner, and an SAP Human Capital Management Consultant.

All members of staff are recruited through robust safer recruitment processes and undergo enhanced DBS checks, right-to-work verification, employment reference checks, and comprehensive onboarding assessments before commencing work.

Care staff complete mandatory and specialist training programmes covering safeguarding adults, medication administration, infection prevention and control, moving and handling, dementia care, Mental Capacity Act and DoLS, equality and diversity, person-centred care, and professional boundaries. Competency assessments are completed before independent practice and are supported by ongoing refresher training, supervision, spot checks, and annual appraisals.

Oak Support Limited utilises the Roundsys digital care management system to support real-time care delivery, electronic care records, medication management, incident reporting, staff monitoring, and family communication where appropriate. This technology enables transparency, accountability, and responsive service delivery while ensuring accurate and timely record keeping.

Our governance framework includes regular audits of care records, medication administration, infection prevention and control practices, staff competency assessments, service user feedback, incident analysis, complaints monitoring, and continuous service improvement planning.

Wherever possible, we provide continuity of care through consistent staffing arrangements, enabling trusting relationships to develop between service users and their care teams.

All care packages are developed following a comprehensive assessment conducted within the individual's home environment. Care plans and contracts are typically issued within 48 to 72 hours of assessment and are reviewed regularly to ensure they remain appropriate and responsive to changing needs.

In addition to personal care, our services include support with hospital appointments, companionship, shopping, social activities, day centres, community engagement, and promoting independence within the home and wider community.
