About Us
New Horizon Care Home Ltd provides supported living services for adults who need personalised support to live safely and as independently as possible in their own home and local community.
We support people with a range of needs, including learning disabilities, autism, mental health needs, complex behaviours, epilepsy, physical health needs and daily living support. Our aim is to help each person build confidence, develop life skills, make informed choices and enjoy a meaningful everyday life.
Our support is person-centred and tailored around each individual’s needs, preferences, communication style, routines, risks and goals. We work closely with the person, their family, advocates, social workers, health professionals and commissioners to make sure support is well planned, safe and coordinated.
We promote choice, independence, dignity, privacy and inclusion. This includes support with personal care, medicines, appointments, health and wellbeing, budgeting, cooking, household tasks, accessing education or day services, social activities and community participation.
Our staff are trained and supported to provide safe, respectful and compassionate care. We place strong importance on safeguarding, positive risk-taking, learning from incidents, medicines safety, infection prevention, health and safety, and continuous improvement.
